Make a Mother’s Day Card that mom will want to save forever with this free printable questionnaire and portrait page!
When I taught Kindergarten, one of my favorite activities was making fill-in-the-blank Mother’s Day cards with my students. The answers that they came up with were so cute and entertaining, especially since they were from the heart!
When my oldest daughter was in Kindergarten, she finally came home with one of these cards too. To this day, it’s my all time favorite Mother’s Day card from her. All her answers were amusing but my favorite was her answer to “Mommy smells like_________”. She answered “peanut butter” to that one and I’ve never laughed so hard! It’s true, I do have a bit of a peanut butter addiction and now the big joke around the house is that I smell like peanut butter! I guess it could be worse, right?
I thought it would be fun to see how my four year old would answer some of these questions so I decided to create one just for her. I’m sharing it with you so that you can have as much fun with it as I did. Since you probably don’t want to see the answers in advance, just pull out the supplies and put daddy in charge! I’ve also included a page where your little one can draw a picture of you so when Mother’s Day rolls around, open your card and start your day with a smile!
You don’t need many supplies, just a 12 x 18 sheet of construction paper, a glue stick and crayons. You can also use stickers, glitter (though I’m not sure about daddy and glitter), sequins and any other crafty decor for the front of the card.
Have your child draw a picture of mommy and answer the fun questionnaire (it’s best to have daddy or an older sibling do it so that the answers will be a surprise to you on Mother’s Day). Fold the construction paper in half and open it back up. Your child can glue the drawing on the left side of the card and the questionnaire on the right, then get creative and decorate the front. We did ours a little early so I could show you the finished product. Keira actually thought it was a test (though I assured her it wasn’t) and after every question, she asked “Did I get it right?”…so cute!!!
Apparently all I do is sit around eating peanut butter and drinking coffee! These really are my favorite cards ever…it’s so fun to hear what your little ones REALLY think about you. I have to admit, it completely melted my heart when she said she loved me because I’m the best mommy ever!
